Clinical notes, and an AI scribe that you stay in charge of
Less admin. More clinic. Structured visit notes, and an AI scribe in the pilot that drafts clinical notes for the clinician to review and sign. A clinician must review every draft before it can become the record.

SOAP notes built for a working day
Visit notes are written in SOAP sections. They autosave as you type, warn you if two people edit the same note, and keep an edit history. A note cannot be signed off until your clinic’s own rules are met.
- Autosave as you type
- Edit-conflict warnings instead of silent overwrites
- Edit history kept with the note
- Sign-off only once your clinic’s rules are met
Sign-off stays with the clinician. Praxia sets the gate; the clinician walks through it.
Intake before the patient arrives
Patients can complete intake forms online before they arrive, and the answers land in their record. The clinician can open the visit with that history already in the record.
Referral and discharge letters, reviewed before they go
Praxia can draft a first-pass referral or discharge letter to a GP. A clinician reviews and approves every draft before anything is sent.
The draft saves the blank-page time. The judgement, and the signature, stay with the clinician.
The AI scribe: part of the pilot, with the clinician in control
The pilot includes an AI scribe that drafts clinical notes for the clinician to review and sign. It is part of the pilot release and is not yet generally available, and we will say so plainly.
It works like this: it drafts a SOAP note from a recorded consult, and a clinician must review the draft before it can become the record. There is no auto-accept.
The review gate is enforced for who can act, not just in the interface. Only a clinician or the clinic owner can accept or discard a scribe draft or play back the consult audio. Reception and other staff cannot.

What we are not claiming about the scribe is set out further down this page, under What we are not saying yet.
Who sees what
Access in Praxia is role based, so what a person can open depends on their role at the clinic. For how records are held, see security and data.
Why the review step matters
A clinical record is the clinician’s account of the consult, written under their name. Anything that drafts text for you, whether a scribe or a letter template, has to leave the decision with the person who is responsible for the record. That is the principle Praxia is built around: drafts are drafts until a clinician has reviewed them.
For notes, that means sign-off is gated by your clinic’s own rules. For letters, it means nothing is sent until a clinician has approved it. For the scribe, it means a draft cannot become the record without a clinician reviewing it first.
What we are not saying yet
There are things we will not claim on this page about the scribe. We do not describe how well it performs, because we have no evidence we are prepared to publish. We do not make statements about regulatory classification. We do not describe where consult audio is handled. If those matter to your clinic, and they should, ask us in a walkthrough and we will tell you what is decided and what is not.
Clinics sometimes ask whether they can wait and see. You can. The structured notes, the intake forms and the reviewed letters do not depend on the scribe, and a clinic can start with them.
Where Praxia is today
Praxia is being piloted with Evolution Medical Care, a clinic run by Praxia’s founders, and is being onboarded to it now. Praxia Clinical Solutions is an Australian company, currently in incorporation.

Notes and scribe questions
Is the AI scribe part of the pilot?
Yes. The pilot includes an AI scribe that drafts clinical notes for the clinician to review and sign. It is in the pilot release and is not yet generally available. Structured notes, intake forms and reviewed letter drafts are part of the pilot too.
Does the scribe write straight into the record?
No. A clinician must review the draft before it can become the record. There is no auto-accept.
Can my reception team accept a draft?
No. Only a clinician or the clinic owner can accept or discard a scribe draft.
Do I have to use the scribe?
No. The notes work on their own without it.
What does it cost?
Praxia is priced per practitioner per month, starting at A$199 (excluding GST) for a solo practitioner, with lower per-practitioner rates as a clinic grows. The full price list is on the pricing page.
